This phase allows for changes to be made to KSB based on the root causes of unsafe KSB |

Step 1: Address
action logs |
|
In this step the steering team needs to attempt to carry out all tasks on action logs by their deadline date.
One way to tackle the actions is to prioritise them according to urgency.
NB> The program often slows pace at this point because much time is allocated to this phase, however, it is very important to make changes as soon as possible, because this will be noticed by others as changes made directly from the information they supplied and it reduce the amount of time where operatives are not involved in the process.
The reason for time allocated for this phase is because a lot of changes will take a long time to get authorisation for or to produce/receive things required. |
|
|
|
Step 2: Prepare Fun Initiatives |
 |
|
In order to involve the operatives in this workforce driven process a lot more than merely through their suggestions and providing them with feed back, enjoyable initiatives can also be employed to serve this purpose and to raise awareness on the programme and the KSB.
These initiatives can be fun for all on site. Examples of such initatives are:
-Posters competitions, drawing a picture related to a KSB or all 20 KSB.
-Personal cross words relating to the KSB.
-Small giveaways, e.g. Coasters with KSB cartoon pictures.
-*more
Further to past case study examples on the right, please see more examples of initiative and raising awareness ideas in the slide show. |

|
Step 3: Involve workforce on decision on initiatives |
 |
|
Ask the rest of the workforce what they want. What are they interested in?
This is best done with members of the steering team talking to people that they usually don't talk to.
At the next steering team meeting come together with ideas brainstormed on site and decide on intiatives to carry out and plan its execution.
Good questions to ask are:
-when will the initiative start?
-what is needed before the initiative can start?
-when the initiative will finish?
-do we need managers approval?
-what needs to be managed throughout the initiative?
-who is responsible for what?

PHASE SUMMARY
| In this phase,
analyses of the results from Phase 3 were used to uncover the underlying reasons for the at-risk KSB. This information will be used to make relevant changes to safety in the next phase. |
